Who Can Make a Referral?
Referrals may be made by:
- Parents and caregivers
- Support coordinators
- General practitioners (GPs)
- Psychologists
- Educators and teachers
- Early childhood educators
- Allied health professionals
We welcome both self-referrals and professional referrals.
How to Make a Referral
There are several ways to submit a referral.
Option 1 — Online Referral Form
Complete our online referral form to help us understand your child’s needs.
Option 2 — Email Referral
You may also send referral information via email.

- Child’s name
- Date of birth
- Parent/caregiver contact details
- Reason for referral
- Relevant reports (if available)
What Happens After a Referral?
Once we receive your referral, our team will:
- Review the referral information
- Contact you to discuss your child’s needs
- Recommend the most suitable service
- Arrange assessment or consultation (if required)
- Confirm service availability
We aim to contact families as soon as possible after receiving a referral.

We currently accept referrals under:
- NDIS (Self-Managed)
- NDIS (Plan-Managed)
- Private Funding
- Medicare

Waiting List Information
Depending on service demand, a waiting list may apply.
We prioritise referrals based on:
